1. Acceptance of Terms

By accessing or using the My4MLife website, mobile application, telehealth platform, or any related services (collectively, the "Services"), you acknowledge that you have read, understood, and agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, you must not access or use our Services.

These Terms constitute a legally binding agreement between you ("User," "you," or "your") and My4MLife ("Company," "we," "us," or "our"). We reserve the right to modify these Terms at any time. Material changes will be communicated via email or a prominent notice on our website. Your continued use of the Services after any modifications constitutes your acceptance of the updated Terms.

These Terms apply in addition to any other agreements you may enter into with My4MLife, including but not limited to telehealth consent forms, medical intake questionnaires, and prescription agreements.

2. Description of Services

My4MLife is a health-education and protocol-coaching platform. Medical care — including diagnosis, prescriptions, lab orders, and clinical decision-making — is delivered by our contracted licensed telemedicine partner, not by My4MLife directly. Our services include:

  • The 4M Framework + 4M App: the integrated education + protocol-coaching system built around Mind, Muscle, Mitigate, Motivate. Includes daily protocol guidance, weekly group Zooms, assessment-driven priority tracking, and content delivery.
  • Coordination with our licensed telemedicine partner: patients who purchase a Comprehensive 4M Consult are connected to the partner's licensed clinicians, who deliver the consult, interpret labs, write prescriptions where medically appropriate, and serve as the prescriber-of-record.
  • OTC nutraceutical products: the proprietary supplement line (Biome NS Ultra, NeuroBridge, SleepRestore, ArmorVita, MitoVita, OmegaCN Prime) sold direct via subscription and one-time bundles.
  • Coordination with our compounding pharmacy partner for fulfillment of prescriptions issued by the telemedicine partner (including Rx add-on products like Biome NS Rx and SleepRestore Rx).
  • Educational Resources: blog content, weekly Zooms, member education on cognitive longevity, brain-health-first lifestyle changes, and the 4M protocol.

The specific services available to you may vary based on your location, medical history, and the licensure of our telemedicine partner. My4MLife does not provide medical care, does not guarantee any treatment or medication will be prescribed, and does not make medical decisions — all clinical decisions are made at the sole discretion of the licensed treating clinician at our partner practice.

3. Eligibility Requirements

To use the Services, you must meet all of the following eligibility requirements:

  • Age: You must be at least 18 years of age. My4MLife does not provide services to minors.
  • Residency: You must be a current resident of the United States and located in a state where our Services are available at the time of your consultation.
  • Medical History: You must provide a complete, truthful, and accurate medical history, including current medications, known allergies, pre-existing conditions, and prior surgical history.
  • Identity Verification: You must provide valid government-issued identification and any additional verification documents as requested.
  • Communication: You must have access to a reliable internet connection and a device capable of supporting video or audio consultations.
Important: Providing false, misleading, or incomplete medical information may result in serious health risks, immediate termination of your account, and potential legal liability. You are solely responsible for the accuracy of all information you provide.

4. Telehealth Consent

By using the Services, you consent to receiving healthcare services via telehealth technology. You acknowledge and understand the following:

  • Nature of Telehealth: Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location.
  • Not a Replacement for In-Person Care: Telehealth consultations are not intended to replace your relationship with your primary care physician or other in-person healthcare providers. You should continue to see your regular healthcare providers for comprehensive medical care.
  • Limitations: Telehealth has inherent limitations, including but not limited to the inability to perform physical examinations, the possibility of technology failures, and the potential for incomplete or delayed information transmission. These limitations may affect the quality of medical decisions.
  • Emergency Situations: The Services are not designed for medical emergencies. If you experience a medical emergency, call 911 or go to your nearest emergency room immediately. Do not rely on the My4MLife platform for urgent or emergent medical care.
  • No Guaranteed Outcomes: No specific medical outcomes or results are guaranteed. Treatment effectiveness varies among individuals based on numerous factors including adherence to treatment plans, lifestyle, genetics, and overall health.
  • Right to Withdraw Consent: You may withdraw your consent to telehealth services at any time by contacting us in writing. Withdrawal of consent may result in the discontinuation of your treatment program.

5. Prescription Medications

You acknowledge and understand the following regarding prescription medications provided through the Services:

Compounded Medications Notice: Compounded medications prepared by compounding pharmacies are not FDA-approved. They are prepared by licensed compounding pharmacies based on a licensed provider's prescription to meet the specific needs of an individual patient. The safety, effectiveness, and quality of compounded medications have not been evaluated by the FDA.
  • Prescription Authority: All prescriptions are issued at the sole discretion of licensed healthcare providers based on their independent medical judgment. My4MLife does not prescribe medications; prescriptions are written by the treating provider.
  • Individual Results: Results from GLP-1 medications and other prescribed treatments vary from person to person. Weight loss outcomes depend on multiple factors, including medication adherence, diet, exercise, metabolism, and other individual health considerations. Past results or testimonials do not guarantee your individual outcome.
  • Side Effects: All medications carry risks of side effects. Common side effects of GLP-1 medications may include nausea, vomiting, diarrhea, constipation, abdominal pain, and injection site reactions. You are responsible for reviewing all medication guides, side effect information, and safety disclosures provided to you.
  • Medication Adherence: You agree to take medications only as prescribed and to report any adverse effects, concerns, or changes in your health to your treating provider promptly.
  • No Sharing or Transfer: Prescription medications are prescribed solely for your use. You must not share, sell, transfer, or distribute your medications to any other person.
  • Storage and Handling: You are responsible for properly storing and handling medications according to the instructions provided, including any temperature and refrigeration requirements.

6. Payment Terms and Refund Policy

6.1 Fees and Payment

By enrolling in a My4MLife program, you agree to pay all applicable fees as described at the time of purchase. Fees may include consultation fees, medication costs, shipping charges, and program subscription fees. All prices are listed in U.S. dollars. We reserve the right to change our pricing at any time; however, price changes will not affect orders already placed or active subscriptions until the next billing cycle.

6.2 Billing

For subscription-based programs, your payment method will be charged automatically on a recurring basis (monthly or as otherwise specified) until you cancel. You authorize My4MLife to charge your designated payment method for all fees incurred.

6.3 Refund Policy

Due to the personalized nature of medical services and compounded prescription medications:

  • Consultation fees are non-refundable once a consultation has been completed.
  • Prescription medications that have been shipped cannot be returned or refunded due to health and safety regulations.
  • If a provider determines that you are not a candidate for treatment during your initial consultation, you may be eligible for a refund of medication costs (excluding consultation fees).
  • Shipping charges are non-refundable once an order has been dispatched.
  • Subscription cancellations must be made at least 48 hours before the next billing date to avoid being charged for the upcoming cycle.

Refund requests should be directed to legal@my4mlife.com. Each request will be evaluated on a case-by-case basis. My4MLife reserves the right to make the final determination on all refund requests.

6.4 Insurance

My4MLife does not currently accept insurance. All fees are the responsibility of the patient. We may provide documentation that you can submit to your insurance provider for potential reimbursement, but we make no guarantees regarding insurance coverage or reimbursement.

6.5 Chargebacks — permanent account ban

Initiating a chargeback with your card issuer instead of contacting our refund team will result in immediate and permanent termination of your My4MLife account. All active subscriptions will be cancelled, app access will be revoked instantly, and you will be prohibited from future purchases on the platform. This consequence is automatic and non-appealable.

Before filing a chargeback, you must first contact refunds@my4mlife.com. We respond within 3 business days and refund eligible orders within 5–7 business days. The chargeback ban applies only when a customer bypasses our refund process to file a chargeback directly. We refund willingly when asked; we ban permanently when bypassed.

By accepting these Terms you acknowledge and agree to this chargeback policy.

6.6 Pricing transparency

My4MLife uses a transparent pricing model. OTC supplement products are available through Amazon affiliate links at standard retail pricing — My4MLife does not control or guarantee Amazon pricing. Rx protocols are priced by the contracted telemedicine and pharmacy partners. Protégé membership is free and does not confer pricing discounts on any products or services. All prices displayed on the platform reflect current retail rates and are subject to change.

7. User Accounts and Responsibilities

When you create an account with My4MLife, you agree to the following:

  • Account Security: You are responsible for maintaining the confidentiality of your account credentials, including your username and password. You agree to notify us immediately of any unauthorized access or use of your account.
  • Accurate Information: You agree to provide and maintain accurate, current, and complete information in your account profile and medical records.
  • Personal Use Only: Your account is for your personal use only. You may not allow any other person to use your account or access your medical information through the platform.
  • Account Activity: You are responsible for all activity that occurs under your account, whether or not you authorized that activity.
  • Communication Preferences: By creating an account, you consent to receive communications from My4MLife, including appointment reminders, treatment updates, and service-related notifications via email, SMS, or push notification. You may opt out of marketing communications at any time.

8. Prohibited Uses

You agree not to use the Services for any of the following purposes:

  • Providing false, misleading, or fraudulent information, including but not limited to medical history, identity, or insurance information.
  • Seeking prescriptions for purposes other than your own legitimate medical treatment.
  • Reselling, redistributing, or transferring medications or prescriptions obtained through the platform.
  • Attempting to obtain medications through deception or by circumventing medical evaluation requirements.
  • Using the platform to harass, abuse, threaten, or intimidate any healthcare provider, staff member, or other user.
  • Interfering with, disrupting, or attempting to gain unauthorized access to our systems, servers, networks, or databases.
  • Introducing malware, viruses, or any other harmful code to the platform.
  • Scraping, data mining, or using automated tools to extract information from the platform.
  • Using the Services in any manner that violates applicable federal, state, or local laws or regulations.
  • Impersonating any person or entity, or falsely claiming an affiliation with any person or entity.

Violation of these prohibitions may result in immediate account termination, forfeiture of any fees paid, and referral to appropriate law enforcement authorities.

9. Intellectual Property

All content, materials, and intellectual property associated with the Services, including but not limited to the website design, text, graphics, logos, icons, images, software, trademarks, trade names, and service marks (collectively, "Content"), are the exclusive property of My4MLife or its licensors and are protected by United States and international copyright, trademark, and other intellectual property laws.

You are granted a limited, non-exclusive, non-transferable, revocable license to access and use the Services and Content for your personal, non-commercial use only. You may not:

  • Copy, reproduce, distribute, or create derivative works from any Content without prior written consent.
  • Use any My4MLife trademarks, logos, or brand elements without express written permission.
  • Remove, alter, or obscure any copyright, trademark, or other proprietary notices.
  • Use any Content for commercial purposes or in any way that may damage the reputation of My4MLife.

The "My4MLife" name, logo, and all related names, logos, product and service names, designs, and slogans are trademarks of My4MLife. You must not use such marks without the prior written permission of My4MLife.

10. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW:

  • My4MLife, ITS OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, AFFILIATES, AND LICENSORS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, INCLUDING BUT NOT LIMITED TO LOSS OF PROFITS, DATA, USE, GOODWILL, OR OTHER INTANGIBLE LOSSES, ARISING OUT OF OR IN CONNECTION WITH YOUR USE OF OR INABILITY TO USE THE SERVICES.
  • My4MLife'S TOTAL AGGREGATE LIABILITY ARISING OUT OF OR RELATING TO THESE TERMS OR THE SERVICES SHALL NOT EXCEED THE TOTAL AMOUNT YOU PAID TO My4MLife IN THE TWELVE (12) MONTHS PRECEDING THE EVENT GIVING RISE TO THE CLAIM.
  • My4MLife IS NOT LIABLE FOR ANY ADVERSE HEALTH OUTCOMES RESULTING FROM YOUR FAILURE TO FOLLOW PRESCRIBED TREATMENT PLANS, DISCLOSE RELEVANT MEDICAL INFORMATION, OR SEEK APPROPRIATE EMERGENCY MEDICAL CARE.

THE SERVICES ARE PROVIDED ON AN "AS IS" AND "AS AVAILABLE" BASIS WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. My4MLife DOES NOT WARRANT THAT THE SERVICES WILL BE UNINTERRUPTED, ERROR-FREE, OR SECURE.

Some jurisdictions do not allow the exclusion or limitation of certain warranties or liabilities, so some of the above limitations may not apply to you. In such cases, My4MLife's liability will be limited to the fullest extent permitted by applicable law.

This section contains important information about the limitations of the content and services provided by My4MLife. Please read it carefully.
  • Not Medical Advice: The information provided on the My4MLife website, blog, educational materials, and marketing content is for general informational purposes only and does not constitute medical advice, diagnosis, or treatment. It is not intended as a substitute for professional medical advice from a qualified healthcare provider.
  • Consult Your Doctor: Always seek the advice of your physician or other qualified healthcare provider with any questions you may have regarding a medical condition, medication, or treatment plan.   • Results and Testimonials: Any testimonials, case studies, or before-and-after results shared on the platform reflect individual experiences and are not guarantees of results. Individual outcomes will vary.

12. Indemnification

You agree to indemnify, defend, and hold harmless My4MLife, its officers, directors, employees, agents, affiliates, successors, and assigns from and against any and all claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys' fees and court costs) arising out of or related to:

  • Your use of or access to the Services.
  • Your violation of these Terms or any applicable law or regulation.
  • Your provision of false, misleading, or incomplete information.
  • Your negligence, willful misconduct, or misuse of prescribed medications.
  • Any claim by a third party arising from your use of the Services.
  • Your violation of any rights of any third party.

This indemnification obligation will survive the termination of these Terms and your use of the Services.

13. Dispute Resolution and Arbitration

13.1 Informal Resolution

Before initiating any formal dispute resolution proceedings, you agree to first contact My4MLife at legal@my4mlife.com to attempt to resolve the dispute informally. Both parties agree to negotiate in good faith for a period of at least thirty (30) days before pursuing arbitration or litigation.

13.2 Binding Arbitration

If the dispute cannot be resolved informally, you and My4MLife agree that any dispute, claim, or controversy arising out of or relating to these Terms or the Services shall be resolved through binding individual arbitration administered by the American Arbitration Association ("AAA") under its Consumer Arbitration Rules. The arbitration shall be conducted by a single arbitrator and shall take place in the state where you reside or, at your election, via telephone or video conference.

13.3 Class Action Waiver

YOU AND My4MLife AGREE THAT EACH PARTY MAY BRING CLAIMS AGAINST THE OTHER ONLY IN YOUR OR ITS INDIVIDUAL CAPACITY, AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED CLASS, CONSOLIDATED, OR REPRESENTATIVE ACTION. The arbitrator may not consolidate more than one person's claims and may not preside over any form of class or representative proceeding.

13.4 Exceptions

Notwithstanding the above, either party may bring an individual action in small claims court for disputes within that court's jurisdictional limits. Additionally, either party may seek injunctive or equitable relief in any court of competent jurisdiction to prevent the actual or threatened infringement of intellectual property rights.

14. Governing Law

These Terms and any disputes arising out of or related to the Services shall be governed by and construed in accordance with the laws of the State of Delaware, without regard to its conflict of laws principles. To the extent that arbitration does not apply, you consent to the exclusive jurisdiction and venue of the state and federal courts located in the State of Delaware for the resolution of any disputes.

15. Termination

Either you or My4MLife may terminate your account and access to the Services at any time, for any reason or no reason, with or without notice.

15.1 Termination by You

You may terminate your account at any time by contacting us at legal@my4mlife.com or through your account settings. Termination of your account does not relieve you of any obligation to pay fees incurred prior to termination.

15.2 Termination by My4MLife

We reserve the right to suspend or terminate your account immediately, without prior notice, if we reasonably believe that:

  • You have violated these Terms or any applicable law or regulation.
  • You have provided false or misleading information.
  • Your continued use of the Services poses a risk to your health or the health of others.
  • You have engaged in abusive, fraudulent, or inappropriate conduct.
  • Continued provision of Services is no longer feasible due to regulatory, legal, or operational reasons.

15.3 Effect of Termination

Upon termination, your right to access and use the Services will cease immediately. Provisions of these Terms that by their nature should survive termination shall survive, including but not limited to Sections 10, 11, 12, 13, 14, and this Section 15. We may retain your medical records as required by applicable laws and regulations.
